An unannounced monitoring inspection was conducted beginning at 1:45 p.m. and ending at 3:15 p.m. with the director. There were 12 children in care ranging in age from 3 to 4 years of age under the care and supervision of 5 staff. Children were observed during quiet movie time. Center documents and nine staff records were reviewed.

Based on review of documentation and interview with staff, it was determined that the center did not have documentation of central registry background check results within 30 days of employment. Evidence: Staff #1, hired 8/19/13, Staff #2, hired 9/13/10, Staff #3, hired 8/24/15, Staff #4, hired 8/19/13, Staff #5, hired 10/1/12, Staff #6, hired 8/22/1995, Staff #7, hired 8/10/18, Staff #8, 8/13/13, Staff #9, hired 8/22/16 did not have documetnation of sworn statment or central registry background checks in the record.
Plan of Correction:
The Diocese was conducting central registry background checks and the results were not available to the facility. Central registry background checks will be conducted on staff. Sworn statements have been completed.
