Technical Assistance:
The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care Programs.
Comments:
This inspection was conducted by the licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia. A monitoring inspection was initiated on 12/14/2020 and concluded on 12/17/2020. The Director was contacted by telephone to initiate the inspection. There were 41 children present with 8 staff. The inspector emailed the Director a list of items required to complete the inspection. The Inspector reviewed 2 children's records and 2 staff records submitted by the facility to ensure documentation was complete. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice. Based on a review of children's records, the provider failed to maintain required documentation for each child enrolled at the center.
Evidence: Child #1's record did not contain complete documentation of name address and phone number for one of the emergency contacts.
Plan of Correction:
I will get the complete address of the emergency contact from the parent.
