Based on record review, the center did not ensure eight of ten staff records had all of the required documentation. Evidence: 1. The record of staff #1 (start date: 11/05/2019) did not have written information to demonstrate the staff possesses the programmatic training required by the job position. 2. The record of staff #4 (start date: 09/05/2019) did not have written information to demonstrate the staff possesses the orientation and programmatic training required by the job position. 3. The record of staff #5 (start date: 09/27/2019) did not have written information to demonstrate the staff possesses the programmatic training required by the job position. 4. The record of staff #6 (start date: 02/18/2014) did not have written information to demonstrate the staff possesses the staff development training required by the job position. 5. The record of staff #7 (start date: 02/25/20119) did not have written information to demonstrate the staff possesses the orientation training required by the job position. 6. The record of staff #8 (start date: 04/23/2019) did not have written information to demonstrate the staff possesses the education required by the job position. 7. The record of staff #9 (start date: 09/05/2019) did not have written information to demonstrate the staff possesses the programmatic training and experience required by the job position. 8. The record of staff #10 (start date: 08/30/2019) did not have written information to demonstrate the staff possesses programmatic training and experience required by the job position.
