Comments:
An announced subsidy inspection was conducted at the administrative building in Richmond on December 13, 2019 to review staff records. Ten staff records were reviewed. If you have any questions, please contact the licensing inspector at (804) 662-9038.
Based on record review, the vendor did not ensure two of ten staff record contained documentation of Virginia Preservice Training within the first 90 days of employment. Evidence: 1. The record of staff #4 (start date: 9/05/19) did not have documentation of Virginia Preservice Training. 2. The record of staff #3 (start date: 6/12/19) did not have documentation of Virginia Preservice Training.
