Comments:
An unannounced monitoring inspection was conducted today from 10:35am-1:30pm. There were 43 children (2-5yrs.) supervised by 6 direct care staff. The physical plant, 3 staff records, 4 children?s records, evacuation drills, injury reports, emergency supplies, and policies were inspected. There were no medications on site today. Children were observed playing outside on the playground, playing educational games on the tablet, painting, and engaging with Legos, puzzles, and blocks. There was a sufficient amount of books, toys, and materials, which were found to be developmentally appropriate for the children in care at the time of inspection. Areas of non-compliance are identified in the Violation Notice.

Description:
Based on observation, the center did not ensure that areas and equipment of the center were maintained in a clean, safe, and operable condition.
Evidence: 1. A blue, plastic, children's wagon on the outdoor playground had a broken safety strap. 2. The metal climber on the outdoor playground had chipped paint. 3. The wooden climber on the outdoor playground had areas of chipped wood and peeling paint.
