This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A renewal inspection was initiated on May 12, 2020 and concluded on May 18, 2020. The director was contacted by telephone to initiate the inspection. There were zero children present and three staff. The inspector emailed the director a list of items required to complete the inspection. The Inspector reviewed two children?s records and four staff records submitted by the facility to ensure documentation was complete.
Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the facility.
On August 4, 2020, the inspection was amended to include out of state child abuse and neglect search for one applicant/agent, ? 63.2-1721.1 B 3.

Based on a review of documentation and interview with the director, the center did not request an out of state child abuse and neglect registry check for one applicant/agent who has lived outside the state of Virginia in the last five years.
Evidence: 1. The record for staff #3 contained a sworn disclosure that indicated staff #3 has resided in another state, Alabama, in the past five years. The record did not contain an out of state child abuse and neglect search for the state of Alabama. 2. The director acknowledged this search request had not been completed.
