Comments:
An unannounced monitoring inspection was initiated on April 22, 2024 and concluded on April 25, 2024. Ten staff were present supervising fifty-two preschool age children. Additional staff were on site for administrative duties. Children were observed playing outside and participating in various activities. Five staff and five child records were reviewed. Violation found is noted on the violation notice.
Based on review, facility did not ensure 4 of 5 staff records have documentation of all required information.
Evidence: The records for staff #1, #2, #4 and #5 did not have documentation of negative TB tests or screenings. Staff records were requested on April 22, 2024 at 11:30 am and as of April 25, 2024 at 11:00, TB information was not submitted.
