Comments:
An unannounced monitoring inspection was conducted today between the hours of 10:00am-11:00am. There were a total of 28 children with four staff members present. Children were observed during hand washing, snack time, engaged in group activities, puzzles playdough, and outdoor play time. A selection of staff and children records, medications and other documents were reviewed. The physical space, emergency supplies, evacuation drills, attendance records, and the Emergency Preparedness Plan were reviewed. Areas of non-compliance are identified in the violation notice. Description: Based on record review, the center does not have a current central registry finding.
Evidence:
1. Staff #2's (Date of hire-08/22/2017) record contained an expired Central Registry finding. The Central Registry finding was dated 03/03/2017.

Plan of Correction: A new Central Registry Search will be completed and mailed. Once the results come back to the school, the results will be emailed to the Licensing Inspector.

Description: Based on record review and interview, the center did not develop written procedures for prevention of shaken baby syndrome or abusive head trauma, including coping with crying babies, safe sleeping practices, and sudden infant death syndrome awareness.
Evidence:
1. There was no documentation of written procedures for prevention of shaken baby syndrome or abusive head trauma, including coping with crying babies, safe sleeping practices, and sudden infant death syndrome awareness.
2. Staff #3 confirmed that written procedures for prevention of shaken baby syndrome or abusive head trauma, including coping with crying babies, safe sleeping practices, and sudden infant death syndrome awareness were not developed.
