Comments:
An unannounced monitoring inspection was conducted today from 11:45am-2:00pm. There were reported to be 80 children (6yrs old - 12yrs old) and 25 staff on a field trip during the inspection. The physical plant, pool area, 6 staff records, 2 lifeguard records, 8 children?s records, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children enrolled in the preschool program were observed participating in group play, playing in the gym area, and playing outside. There was an abundant supply of materials for the children. Areas of non-compliance are identified on the Violation Notice. Standard #: 22VAC40-185-60-A
Description: Based on record review, children's records were incomplete. Evidence: Child#2's record is missing the child's father work telephone number. Child#3's record is missing the child's mother work telephone number. Child#6's record is missing the parents' work telephone number.

Plan of Correction: We have reached out to all the families for correct information and are changing our processing system to ensure all information is captured.

Standard #: 22VAC40-185-240-A
Description: Based on record review, one out of 6 staff records was missing documentation of receiving the required training by the end of their first day of assuming job responsibilities. Evidence: Documentation of orientation training was not available at the time of the inspection for Staff#4.

Plan of Correction: We reached out to the staff member to get a copy of the training sheet. The training was completed on time.

Standard #: 22VAC40-185-280-B
Description: Based on observation, hazardous substances were not kept in a locked place suing a safe locking method that prevent access by children. Evidence: In the restroom that leads to the pool, a bottle of Liquid Fog, labeled "keep out of reach of children", was observed on the floor unlocked and accessible to children.

Plan of Correction: It was immediately removed.

Standard #: 22VAC40-185-480-B
Description: Based on record review, children's files did not contain a written swimming permission from the parent of each child who participates in swimming or wadding. Evidence: Children's records reviewed did not include a swimming permission form from the parents authorizing their children to participate in swimming activities, only the children's swimming abilities were obtained.

Plan of Correction: We will change the summer permission agreements to include the more specific language.
