Comments:
The Licensing Inspector initiated a monitoring inspection on 09/29/2022 and completed it on 9/30/2022. The inspector was on site from 10:45am until 1:10pm on 09/29/2022 and 8:30am until 9:40 on 09/30/2022.

There were 111 children under the care of 16 staff. The ages of the children ranged from 4 months to four years old. The children were observed during free play, outside time, meal time and nap time. Ten children?s records and six staff records were reviewed.


Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program

Violations:
Standard #: 8VAC20-780-270-A
Description: Based on observation, the center failed to ensure the playground was clean, safe and operable.

Evidence: Inspector observed resilient surfacing turf separating on the playground. The turf had separated approximately 2 inches in some area causing a tripping hazard for children

Plan of Correction: We are going to fill the holes and put carpet over this to prevent the turf separating.

Standard #: 8VAC20-780-440-K
Description: Based on observation, the center failed to ensure crib sides shall be up and fastenings secure when a child is in the crib.


Evidence: Inspector observed a crib with a screw sticking out approximately an inch. This screw was meant to secure the siding of the crib. There was a sleeping child in the crib.

Plan of Correction: Crib was fixed the next day.
