Comments:
A self-report was received by the Office of Child Care Health and Safety on 5/23/2023. An unannounced inspection was conducted 5/30/2023. The inspector was on site from 4:10pm to 5:15pm.
The information gathered during the inspection did not support the self-report and therefore the self-report was determined to be invalid. Information gathered during the course of the investigation determined non-compliance with applicable standards or law. Violations unrelated to the complaint were documented on the violation notice issued to the provider.
Based on record review, the center failed to renew the Central Registry check after expiration of 5 years.
Evidence: Staff #1 had a Central Registry on file dated for 5/2/2018. A new Central Registry check not available during the inspection on 5/23/2023.
Staff #2 had a Central Registry on file dated for 5/9/2018. A new Central Registry check was not available during the inspection on 5/23/2023.
Plan of Correction:
Fingerprints were scheduled by the end of the month.
