Comments:
An unannounced monitoring inspection was initiated and completed on 12/5/2023. The inspector was on site from 10:00am until 1:20pm.

There were 123 children, ages 3 months to 5years old under the supervision of 25 staff. Children were observed during indoor free play, lunchtime, outdoor play and nap. Eight children's records and nine staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-780-270-A
Description: Based on observation, the center failed to ensure areas and equipment of the center, inside and outside, shall be maintained in a clean, safe and operable condition.

Evidence:
1.On the playground, the resilient surfacing is separating causing gaps in the surfacing which cause a tripping hazard.
2. In the preschool 2 classroom, the water fountain spout has become slightly detached from the sink causes a hole where it should be. This creates a pinching hazard to children.
3.In the Preschool classroom, the wood panels have come apart creating a gap in between pieces. This creates a pinching hazard for children.

Plan of Correction: 1. Contacting a playground specialist to see what other options we have. 12-15
