The following was discussed with the program director during the exit interview:

The importance of transitioning activities and how to implement them during busy times of the day.

Comments:
A monitoring inspection was conducted on 05/09/2022 An in-person tour of the facility was conducted in addition to a record review of a list of items required to complete the inspection. The Inspector reviewed 5 children?s records and 7 staff records, along with any requested program records submitted by the facility to determine if required documentation was complete. Children were observed finishing lunch and preparing for nap. Infants were observed playing on the floor, sleeping in cribs and being fed.

Violations:
Standard #: 22VAC40-185-270-A
Description: Based on observation the center did not ensure al areas of the center, inside and outside, were maintained in a safe condition.
Evidence: There were pointy gumballs from the trees on the playground and grassy surfaces where children play.

Plan of Correction: We will partner with our facilities team to ensure that mulch is replenished on or before June 30, 2022. Moving forward, the safety captain will measure mulch in the fall zones to ensure compliance. School leadership will also check for this criterion monthly

Description: Based on observation the center did not ensure where playground equipment is provided, resilient surfacing shall comply with minimum safety standards when tested in accordance with the procedures described in the American Society for Testing and Materials standard F1292-99.
Evidence:
The center's 2 large play structures did not have at least 6 inches of resilient surfacing within the fall zones.

Plan of Correction: We will work with our landscaping company to get the gumballs currently present removed on or before June 30. Our safety captain will notify school leadership when excessive gumballs are present to request removal services from our landscaper. Staff will ensure that all sidewalk areas are swept daily to avoid trip hazards.
