Comments:
A monitoring inspection was conducted on 04/05/2023. A tour of the facility was conducted, in addition to a record review and interviews with staff. The inspector emailed the director/provider a list of items required to complete the inspection. Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-780-60-A
Description: Based on a record review the center did not ensure all required information was kept for each enrolled child.
Evidence: Child #2, enrolled 07/25/2022, did not have documentation of having a physical examination under the direction of a physician.

Plan of Correction: This was an administrative oversight. The director will contact the parent today and request a copy of the child's physical. It will be documented and kept in the child's file.

Standard #: 8VAC20-780-280-B
Description: Based on observation the center did not ensure hazardous substances were kept in a locked place using a safe locking method that prevents access by children.
Evidence:
In the preschool classroom there was an unlocked cabinet that contained disinfectant wipes.

Plan of Correction: The wipes were moved to a locked area. Additional safety locks have been ordered for the entire center.

Standard #: 8VAC20-780-330-B
Description: Based on observation the center did not ensure where playground equipment is provided, resilient surfacing shall comply with minimum safety standards when tested in accordance with the procedures described in the American Society for Testing and Materials standard F1292-99.
Evidence:
There were 2 composite playground structures that had approximately 3 inches of resilient surfacing in the fall zones, where at least 6 inches is required.

Plan of Correction: The director placed a work order for additional mulch during the inspection. Corporate expedited the order. The center will rake the mulch that is in place in the high frequency fall zone areas.
