An investigation was initiated from 1:30 pm - 3:00 pm in conjunction with a monitoring inspection to conduct an investigation regarding a self-reported incident involving supervision. Interviews were conducted, documentation, children's records, and staff records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Based on interviews it was determined that children under 10 years of age did not always remain in sight and sound supervision of staff. Evidence: On 09/13/2023 two staff were outdoors on the playground with three children. At approximately 5:58 pm the toddler classroom was lining up to come inside from the playground. Child A (Age 20 months) was left outside on the playground. Approximately three minutes later staff heard the child crying on the playground and retrieved the child.
