Comments:
An unannounced monitoring inspection was conducted on 10/12/2023 from 1:00 pm- 2:15 pm. There were 35 children supervised by 6 direct care staff. The physical plant, 5 staff records, 5 children?s records, medications, evacuation drills, emergency supplies, and policies were inspected. Children were observed playing outside. Hand-washing procedures were also observed. Areas of non-compliance are identified in the Violation Notice. Please submit your plan of correction within 5 business days for it to be included on the website. Thank you for your cooperation during this inspection. Based on record review and interview, the center did not obtain the results of an out-of-state central registry check for all states employees have resided in within the previous five years.
Evidence: Staff #2's (DOH: 8/12/2023) record did not contain a central registry check results from a state they have listed as residing in within the previous five years.
