A self-reported investigation concerning forbidden actions was initiated on 03/13/2024, after the center reported an allegation to the Fairfax Licensing office on 03/13/2024. The investigation was conducted on 03/13/2024 and 03/20/2024, and completed on 03/28/2024. The investigation included interviews with staff members a review of 1 child record, and 1 staff record.
