Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and a virtual tour of the program.
A monitoring inspection was initiated and concluded on 9/13/2021. The director was contacted by telephone and a virtual inspection was conducted. There were 32 children present, ranging in ages from 3 years old to 5 years old, with 6 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 3 child records and 3 staff records were reviewed.

Based on record review, children's records did not all contain required information. Evidence: 1. Child #2 was missing both parent's place of employment and one work phone number. 2. Child #3 was missing both parent's work phone numbers.

I will ask both parents to provide their place of employment and work numbers.

Description:
Based on record review, required records were not kept for each staff person. Evidence: 1. Staff #3 date of hire (8/29/2006) did not have two references documented in their file with the date of contact, the results and signature of person making the call.
