Comments:
An unannounced monitoring inspection was conducted on 05/02/2024. There were 101 children, ages 3 months - 5 years, 27 staff members providing direct care and supervision and various support staff present during the inspection. The inspector reviewed compliance in the areas of administration, staff qualifications and trainings, physical plant, staffing and supervision, programs, emergencies, and nutrition. The children were observed during arrival, outdoor time, mealtime, teacher directed activities and child directed activities. The inspector discussed the following with the provider: changing tables and MAT Training. A total of 10 children?s records, 10 staff records, and 5 medications were reviewed. The inspector arrived for the inspection at approximately 8:20 am and departed at approximately 11:20 am.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the provider.
REPEAT VIOLATION Based on observations, the center failed to ensure that hazardous substances such as cleaning materials, insecticides and pesticides shall be kept in a locked place using a safe locking method that prevents access by children.
Evidence: An aerosol can of ?Lysol? was located on the changing table in The Toddler 2 room. Plan of correction: Lysol was removed and locked during inspection.

Description:
Based on observation and record review, the center failed to ensure that the required staff to child ratio shall be maintained for 2-year-olds.
Evidence: At approximately 8:42 am., there were 9 children observed with 1 staff member, Staff 11. Staff 11 confirmed that 9 children had been in their care for approximately 15 minutes. The youngest child was 2 years old. The required staff ratio for this group of children was 1:8. Two staff would be required.

A second staff member arrived during the inspection. Staff will notify administration staff as soon as they are out of ratio.
