Comments:
The licensing inspector conducted an unannounced monitoring inspection on 1/14/2020 from 10:10am to 2:15pm. The children were observed learning through play in centers, participating in story time, playing instruments, listening to music, dancing and eating lunch. A formal observation was completed in Head start and threes and fours classrooms. lunch consisted of teriyaki chicken, broccoli, pineapples, milk. The menu was posted. Interviews were held with staff throughout the inspection, and the inspector interacted with children in each classroom when appropriate. All classrooms were inspected today.
Five children?s records and six employee records were reviewed during this inspection. Medication was reviewed by the inspector. Five injury reports were reviewed as well as the center?s injury prevention plan, which was updated 12/2019.

Today, the following child to staff ratios were observed: Infant and Toddlers - 12:3 Twos - 7:2 Twos and Threes - 5:1 Twos and Threes - 7:2 Threes - 12:2 Threes and Fours - 8:1 Fours and Fives - 14:2 School Age - 6:1
The violations from the previous inspections were checked for corrections. There were no repeat violations found during today?s inspection.

Based on observation, the center did not ensure children under 10 years of age always shall be within actual sight and sound supervision of staff. Evidence: The inspector observed a staff member assisting children (Age 2) in two separate restrooms while five children (Age 2) stood in the hallway. The staff member entered both restrooms to assist children, leaving the five remaining children in the hallway out of sight supervision. Children under 10 years of age always shall be within actual sight and sound supervision of staff, except that staff need only be able to hear a child who is using the restroom.
Plan of Correction:
If there are two teachers in the classroom, they will both have to be apart of the bathroom breaks or they will take one child at a time if ratio permits. Otherwise, the teachers will call the office for assistance.

Based on Observation, the center did not ensure to follow the most recent, age appropriate nutritional requirements of a recognized authority such as the Child and Adult Care Food Program of the United States Department of Agriculture (USDA). Evidence: The inspector observed the children eat lunch. Lunch consisted of teriyaki chicken, broccoli, pineapples and milk. The children were not served the required grain or bread component.
