On 2/24/2020 one inspector conducted a mandated monitoring inspection (11:15AM to 2:05PM). 32 participants were in care with six direct care staff . Three participant records and two staff records were reviewed. A physical plant tour was done. Lunch and activities were observed. New standards were discussed.
