Technical Assistance:
Note: the facility is currently closed due to the coronavirus emergency.

Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

A monitoring inspection was initiated on 6/8/2020 and concluded on 6/10/2020. The director or in-charge person was contacted by telephone to initiate the inspection. There were no participants present and one staff person present. The inspector emailed the director/person in charge a list of items required to complete the inspection. The inspector reviewed three participant's records and four staff records submitted by the facility to ensure documentation was complete.

Information gathered during the inspection determined non-compliances with applicable standards or law and violations were documented on the violation notice issued to the facility. The Licensing Inspector has reviewed with the director/person in charge COVID-19 guidance posted on the ADCC webpage.

Violations:
Standard #: 22VAC40-61-220-E
Description: Based on participant record review, the facility failed to review and update a participant's assessment every six months.

EVIDENCE:

1. The assessment for participant 2 was dated 8/27/2019.

Plan of Correction: The assessment will be updated when the center is in operation again (closed 3/25/2020). The due date for the assessment fell through the cracks because staff was busy with new procedures to follow during the coronavirus emergency. It is anticipated that the center will reopen in July 2020.

Description: Based on participant record review, the facility failed to review and update a participant's Plan of Care (POC) every six months.

EVIDENCE:

1. The POC for participant 2 was dated 8/27/2019.
