Comments:

A monitoring inspection was initiated on 9/27/2021 and concluded on 9/30/2021. The Director was contacted by telephone to initiate the inspection. The Director reported that the current census was 16. The Inspector emailed the Director a list of items required t complete the remote documentation review portion of the inspection. The Inspector reviewed 2 participant's records, 2 staff records, medication administration records, physician's orders, medications, staff schedule, criminal background check, sworn disclosure statements, menus, activity calendar, fire and health inspections, fire drills submitted by the to ensure documentation was complete. An exit interview was conduced with the Administrator on the date of inspection where findings were reviewed and an opportunity was given for questions, as well as for providing any information or documentation which was not available during the inspection. The information gathered during the inspection determined no violations with applicable standards or law. No violations were issued.
