Comments:

This inspection was conducted by licensing staff using an alternate remote protocol necessary due to a state of emergency health pandemic declared by the Governor of Virginia. A renewal inspection was initiated on 09/21/2020 and concluded on 09/28/2020. The Center Manager was contacted by telephone to initiate the inspection. The center manager reported that the current census was 19. The inspector emailed the center manager a list of items required to complete the inspection. The inspector reviewed 2 resident records, 2 staff records, the current staff schedule, the most recent fire and health inspection reports, fire and emergency evacuation drills, and the activity schedule for the current month submitted by the facility to ensure documentation was complete. Information gathered during the inspection determined non-compliance(s) with applicable standards or law, and violations were documented on the violation notice issued to the facility.

Description:	Based on review of staff records, the facility failed to maintain personal and social data on two staff members. EVIDENCE: 1. The employee records for staff #1 and staff #2 did not include personal social data for either staff member.
