Technical Assistance:
A completed Renewal Application must be submitted prior to the expiration of the current license. An application can be obtained from the DSS website. Comments:
An unannounced renewal study was conducted on 6/13/2023. At the time of entrance nine participants were present with four center staff providing supervision as well as the Director and Assistant Director. This far exceeds the required ratio of one staff per six participants. The sample size consisted of two participant records, two staff records and one individual interview. Participant and staff records and other documentation reviewed. Criminal Background Checks of all staff hired since previous inspection conducted on 4/5/2022 were reviewed. Participants were observed engaging in activities including current events, picture activity and exercise. Exit interview held.

The evidence gathered during the inspection determined no violations with applicable standard(s) or law. The inspection summary will be posted to the VDSS website within five (5) business days of your receipt of the inspection summary.

The department's inspection findings are subject to public disclosure.

Please Note: A copy of the findings of the most recent inspection are required to be posted on the premises of the facility.
