Type of inspection: Monitoring Date(s) of inspection and time the licensing inspector was on-site at the facility for each day of the inspection: 10/31/23 (9:20 AM - 1:45 PM) At the time of entrance, 18 participants were present. Participant to staff ratio was observed. Building and grounds were inspected. Participant records and staff records were reviewed. The sample size consisted of six participant and three staff records. Virginia State Police background checks were reviewed for all new staff hired since the previous inspection. Participants were observed eating lunch and engaging in activities. Medication administration was observed and interviews were conducted. An exit meeting was held to review the inspection findings.
