Comments:

A monitoring inspection was initiated on 11/01/2021 and concluded on 11/10/2021. The program Director was contacted by telephone to initiate the inspection. The program Director reported that the current census was 8. The inspector emailed the program Director a list of items required to complete the remote documentation review portion of the inspection. The inspector reviewed 2 participant, records, 2 staff records, activities calendar, staff schedules, etc. submitted by the program to ensure that documentation was complete. The inspector observed twenty-one participants and five staff on site on 11/10/2021. An exit interview was conducted with the program Director on the date of the on-portion of the inspection, where findings were reviewed and an opportunity was given for questions. The photograph taken of the noncompliance was viewed by the program Director and discussed during the exit interview. The information gathered during the inspection determined noncompliance with applicable standards or law, and violations were documented on the violation notice issued to the facility. Description:	Based on observation the program has failed to ensure that the exterior of all buildings are maintained in good repair. Evidence: As evidenced by the photograph taken on 11/10/2021 the frame of the door that leads to an entrance of the building (off the ramp) was observed to have peeling white paint that exposed dark wood at the base of the door frame.
Plan of Correction:	DIRECTOR'S RESPONSE: "Adult Day Center of Virginia has cleaned the water stained area at the entry door. Gerry Rawlinson , Director will monitor the building for cleaning and maintenance"
