On 06/21/2022 between the approximate times of 2:20p.m until 3:00p.m the inspector was on-site at the day program to investigate allegations regarding Administration and Personnel. The evidence gathered during the investigation did not support the allegations of non-compliance with standard(s) or law. However, violation(s) not related to the complaint but identified during the course of the investigation can be found on the violation notice. The licensee has the opportunity to submit a plan of correction to indicate how the cited violation(s) will be addressed in order to return the facility to compliance and maintain future compliance with applicable standard(s) or law.

Description:	Based on the interview with the site Director the day program failed to ensure that a written discharge notice identified the reasons for discharge and outline the services needed by the participant upon discharge. The discharge notice must be provided to and discussed with the participant and family members or legal representative. Evidence: Participant #1-date of discharge 04/21/2022 During the onsite investigation on 06/21/2022 the site Director did confirm that the participant had been discharged from the day program but did not submit documentation that a written discharge notice was provided to the participant or legal representative.
