Comments:

Two licensing inspectors conducted a one day monitor inspection at Giles Adult Day Services on 02/14/2022. The inspection began at 12:30pm and concluded at 1:50pm. The licensing inspectors observed the center, participant and staff records, the activity calendar and menu. An exit meeting was held with the administrator of the facility. Description:	Based on observations made during the tour of the center, the center failed to have the adequate supplies in each restroom at all times. EVIDENCE: 1. The bathroom used by participants located within the rest area did not have any liquid soap available for hand washing.
Plan of Correction:	Liquid hand soap was placed in bathrooms. A wall dispenser will be purchases. Floor and cleaning staff will also be reminded that bar soap is not permitted. [sic]
