Two licensing inspectors conducted an unannounced mandated renewal inspection at Generations at Haysi on 01/08/2020. This inspection started at 10:45 am and concluded at 11:32 am. The center reported 15 participants enrolled with 9 participants attending on the day of inspection with 7 staff providing care and services. Participants and staff records were reviewed, a tour of the building was conducted, activities were observed throughout the morning, staff and participants interactions were observed and the noon meal was observed. An exit meeting was held with the center director on 01/08/2020. An opportunity was given during the exit meeting to find items not available during the inspection. The findings of the inspection were reviewed. As a result of this inspection 3 violations are being cited. Please complete the "description of action to be taken" and "date to be corrected" for each violation cited on the violation notice, and then return the signed and dated copy back to the licensing office within 10 calendar days (01/19/2020) of receipt. Standard #: 22VAC40-61-330-G-4 Description: Based on observations made during the tour of the building and staff interviews, the facility failed to substitute one activity for another and note the change on the calendar.
EVIDENCE:
1. The activities calendar for 01/08/2020 had ?cranium crunches? for 11:00 am. At 11:07 am, the participants were observed to be coloring.
2. Licensing Inspector interviewed Staff # 1 who stated ?these folks here at this center would not know or understand what we were doing?.Plan of Correction: Educated staff that if one activity is substituted for another, the change shall be noted on the schedule. [sic]
Standard #: 22VAC40-61-360-B-1 Description: Based on observations made during the tour of the building, the facility failed to record the menu substitutions on the posted menu.
EVIDENCE:
1. The monthly menu for January 2020 had BBQ chicken thighs, broccoli, brown rice, carrots, strawberries, and 1 slice of bread listed for 01/08/2020. The white dry erase board had the date of 01/08/2019 written on it and had lasagna, tossed salad and strawberries listed for today?s lunch.
2. The licensing inspector observed lasagna, tossed salad and strawberries being served.Plan of Correction: Educated staff, especially those preparing and serving foods, that any substitutions need to be reported to the supervisor and also indicate substitutions written on all menus for participants to see. [sic]
Standard #: 22VAC40-61-410-E Description: Based on observations made during the tour of the building, the facility failed to keep cleaning products in a locked place when not in use.
EVIDENCE:
1. A 128 ounce Purex laundry detergent, one gallon of bleach, and Bounce fabric softener sheets (80 count) were found to be sitting on the top of the dryer. The washer and dryer are in a closet are, but this area is not locked and is not able to be locked.Plan of Correction: Corrected during inspection. Educated all staff on regulations of cleaning supplies. Cleaning products and harmful materials will be kept in a locked place when not in use. [sic]
