Comments:

An unannounced monitoring inspection was conducted on 12/19/2022. At the time of entrance there were four participants in care and two staff members providing supervision. The Licensing Inspector completed a tour of the physical plant that included the building and grounds of the facility. Two participant and one staff record were reviewed. No participants received medications during the inspection. Activities observed included current events, morning circle and morning exercise. No new staff have been hired since the previous inspection conducted on 11/5/2021. An exit meeting was conducted to review the inspection findings. The evidence gathered during the inspection determined no violations with applicable standard(s) or law. The inspection summary will be posted to the VDSS website within five (5) business days of your receipt of the inspection summary.
