A renewal inspection was completed on March 11, 2022, from 12:45 p.m. to 3:59 p.m. During the entrance conference, the administrator reported that the current census was thirty nine (39). The inspector reviewed eight (8) resident records, six (6) staff records, the activities calendar, staff schedules, training records, M.A.R.s, UAIs, participant service plans, and healthcare logs provided by the facility to ensure documentation was complete. The information gathered during the inspection determined no violations with applicable standards or law. No violations were issued.
