Type of inspection: Monitoring Date(s) of inspection and time the licensing inspector was on-site at the facility for each day of the inspection: The inspection began at approximately 10:30 AM and ended at approximately 1:00 PM. Number of participants present at the facility at the beginning of the inspection: Forty-One The licensing inspector completed a tour of the physical plant that included the building and grounds of the facility. Number of participant records reviewed: Six Number of staff records reviewed: Four Number of interviews conducted with participants: Two Number of interviews conducted with staff: Two Observations by licensing inspector: During the tour of the facility, participants were observed engaging in group and individual activities led by employees. Participants were also observed interacting with other participants and employees instead of engaging in a scheduled activity. Interactions between participants and employees were appropriate. Communal restrooms, sensory stimulation rooms, and dining area were observed. There were no health or safety issued noted. Additional Comments/Discussion: No additional comments. An exit meeting will be conducted to review the inspection findings. The evidence gathered during the inspection determined no violations with applicable standard(s) or law.
