This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia. A monitoring inspection was initiated on 05/27/2021 and concluded on 05/27/2021. The director or in-charge person was contacted by telephone to initiate the inspection. There were 16 participants present and 8 staff. The inspector emailed the director/person in charge a list of items required to complete the inspection. The Inspector reviewed 2 participant?s records and 2 staff records submitted by the facility to ensure documentation was complete. Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the facility. The Licensing Inspector has reviewed with the director/person in charge COVID-19 guidance posted on the ADCC webpage.

Standard #: 22VAC40-61-260-B Description: Based on a review of participant records, the facility failed to ensure that all required information was included on participant physical examinations.
EVIDENCE:
1. The physical examination dated 10/20/2020 in the record for participant 2 is incomplete as it does not have documentation of a statement that specifies whether the participant is considered to be ambulatory or non-ambulatory or a statement that specifies whether the participant is or is not capable of self-administering medication.Plan of Correction: The Director will contact the participants physician to obtain the required information and will discuss physical examination documentation issues with the appropriate entities.
