Comments:

This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia. A renewal inspection was initiated on 2/4/21 and concluded on 2/8/21. The director or in-charge person was contacted by telephone to initiate the inspection. There were 0 participants present and 0 staff. The inspector emailed the director/person in charge a list of items required to complete the inspection. The Inspector reviewed 2 participant?s records and 2 staff records submitted by the facility to ensure documentation was complete. Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the facility. The Licensing Inspector has reviewed with the director/person in charge COVID-19 guidance posted on the ADCC webpage.

Standard #:	22VAC40-61-220-D
Description:	Based on record review and discussion, the facility failed to ensure the assessment include at minimum a description of the participant?s: mental status, including any intellectual, cognitive, and behavioral impairment and known psychiatric or emotional problems. Evidence: 1. Participant #1?s assessment dated 1/22/2020 documented the mental health problems as forgetful; however, the plan of care dated 1/3/2020 documented assistance with memory due to a diagnosis of Schizophrenia. 2. Participant #2?s assessment dated 1/8/2020 documented no mental health problems; however, the plan of care dated 12/9/2019 documented a diagnosis of Dementia. 3. Staff #3 acknowledged that the aforementioned assessments for Participant #1 and Participant #2 had conflicting and/or erroneous information.
Plan of Correction:	1.Tina's World of Love will ensure that participant assessment and plan of care to include all description noted on physical examination and information provided by family members of the participant's: mental status, including any intellectual, cognitive, and behavioral impairment and known psychiatric or emotional problems. 2. Tina's World of Love will ensure that participant plan of care and participant assessment correlate with patient diagnosis.

Description:	Based on record review and discussion, the facility failed to ensure the plan of care included descriptions of identified needs, dates identified, expected outcomes or goals, activities and services and dates of outcomes or goal achieved. Evidence: 1. Participant #1?s assessment dated 1/22/2020 documented a ?no sugar diabetic? diet, physical examination report dated 3/7/2019 documented a low cholesterol, low sodium diet; however, the plan of care dated 1/3/2020 did not include the diet documented on the assessment. 2. Participant #2?s assessment dated 1/8/2020 documented a ?diabetic? diet; however, the plan of care dated 12/9/2019 did not include the ?diabetic? diet. 3. Staff #3 acknowledged the aforementioned plans of care did not include the participant?s diets.
Plan of Correction:	1. Tina's World of Love will ensure participants plan of care included descriptions of identified needs, dates identified, expected outcomes or goals, activities and services and dates of outcomes or goal achieved and this to include any special diets noted by doctor. 2. Tina's World of Love will ensure that participant plan of care and participant assessment correlate with patient diagnosis.
