Comments:
An unannounced monitoring inspection by two Licensing Inspector was conducted on 02/28/2022 from 10:02 a.m. until 1:00 p.m. There were five participants present. One direct care staff, the assistant director, and the program director were present. Three participant records and two staff records were reviewed. No new staff had been hired since the last inspection. A tour of the center was conducted, facility postings were observed, and center documentation was reviewed. The catered lunch meal was also observed. No medications are administered at the center at this time.

Information gathered during the inspection determined non-compliance(s) with applicable standards or law, and violations were documented on the violation notice issued to the facility.

Violations:
Standard #: 22VAC40-61-230-D
Description: Based on record review, the facility failed to ensure the plan of care included descriptions of identified needs, dates identified, expected outcomes or goals, activities and services and dates of outcomes or goal achieved.

Evidence:

1. Participant #1?s Assessment dated 9/16/2021 documented the need for supervision with walking and transferring; however, the POC dated 9/16/2021 states the participant will be given physical assistance with walking and transferring as needed. Participant #1 was also observed on-site at the center during the inspection on 2/28/2022 utilizing the assistance of a cane which was not listed on their assessment or POC.

2. Participant #2?s Assessment dated 2/9/2022 documented the need for supervision and mechanical help with walking and transferring; however, the POC dated 2/9/2022 states the participant will be given physical assistance with walking and transferring as needed. The Assessment also states Participant #2 requires supervision with toileting; however, the POC states the participant will ?be physically assisted? as well. An allergy to ice cream was also noted on Participant #2?s physical exam and other admitting information provided by family; however, this was not addressed on the POC.

Plan of Correction: Going forward the agency will make sure a reassessment and a new POC will be prepared when it is noted that a participant needs have changed.

Agency will make sure both the assessment and the POC matches the needs of the participant. All information provided by participant and family members will be addressed on the POC.

Standard #: 22VAC40-61-460-H
Description: Based on observation and discussion, the center failed to ensure hot water at taps available to participants are maintained within a temperature range of 105?F to 120?F.

Evidence:

1. During a tour of the center, the hot water temperature were checked in two bathrooms utilized by participants. The temperatures in the two bathrooms read 133?F and 135?F.

2. Staff #1 acknowledged the hot water was not within a temperature range of 105?F to 120?F.

Plan of Correction: Agency contacted owner of building and he said he would be able to have the water temperature adjusted by the end of next week.

Staff will periodically check the water with a thermometer to make sure the water temperature is in compliance with regulations.
