Comments:

An initial inspection was initiated on 08/02/2021 and concluded on 08/03/2021. The center director was contacted by telephone to initiate the inspection. The center director reported that the current status is 0. The inspector emailed the center director a list of items required to complete the remote documentation review portion of the inspection. The inspector conducted the on-site portion of the inspection on 08/03/2021. An exit interview was conducted with the center director and assistant director on the date of inspection, where findings were reviewed and an opportunity was given for questions, as well as for providing any information or documentation which was not available during the inspection. Information gathered during the inspection determined non-compliance(s) with applicable standards or law, and violations were documented on the violation notice issued to the facility.

Standard #:	22VAC40-61-300-B
Description:	Based on review of the medication management plan, the facility failed to have readily accessible as reference materials for medication aides at least one pharmacy reference book, drug guide, or medication handbook for nurses that is no more than two years old. EVIDENCE: 1. The facility did not have reference materials on site on the day of inspection.
Plan of Correction:	Purchased on 08/03/2021; will be on-site 08/09/2021 [sic]
Standard #:	22VAC40-61-430-C-4
Description:	Based on observations made during the tour of the building, the facility failed to have at least one moveable thermometer available for measuring temperatures in individual rooms that do not have a fixed thermostat that shows the temperature in the room. EVIDENCE: 1. The facility did not have a moveable thermometer on site to measure the temperature in rooms that did not have a fixed thermostat.
Plan of Correction:	Tp put in place per regulation. Was placed on 08/04/2021 and a photo was sent to the inspector on 08/5/2021 [sic]

Description:	Based on observations made during the tour of the building, the facility failed to have areas equipped with appropriate seasonal outdoor furniture. EVIDENCE: 1. The facility did not have any outdoor furniture in place on the day of the inspection.

Standard #:	22VAC40-61-460-A
Description:	Based on observations made during the tour of the building, the facility failed to have one toilet that is suitable to accomodate a participant who needs human assistnace or specialized equipment available for every 10 participants, or portion thereof. EVIDENCE: 1. In both the men and women restrooms there were a total of three toilets each. Only two of those toilets in each bathroom were equipped with grab bars. The facility is requesting to be licensed for 24 residents, therefore all three toilets in each bathroom needs to be equipped with grab bars.

Description:	Based on observations made during the tour of the building, the facility failed to have an emergency meal and a supply of water to all participants in the event that meals are not able to be prepared or participants are required to shelter in place for a period of time. EVIDENCE: 1. The facility did not have emergency meals or a supply of water on site for all participants in the event that meals are not able to be prepared or participants are required to shelter in place for a period of time.
