Comments:

One licensing inspector conducted an unannounced mandated renewal inspection on 02/07/2022; starting at 11:42 am and concluding at 12:50 pm. The center reported there are 4 participants enrolled with 1 participant attending and 2 staff providing care and services on the day of the inspection. A sample of participant and staff records were reviewed, the building was observed, lunch was observed, and interaction between staff and participants was observed.
