An initial inspection was conducted by the licensing inspector on March 3, 2022. A tour of the center was conducted and measurements were taken. Center postings were observed and sample participant and staff records were reviewed, as well as several policies. The violations cited are identified in this report. Please complete the "plan of correction" and "date to be corrected" for each violation cited on the violation notice and return it to the licensing office within 10 calendar days. Please specify how the violation will be corrected. The plan must contain: 1) step(s) to correct the non-compliance with the standard(s), 2) measures to prevent the non-compliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measure(s). Standard #: 22VAC40-61-440-B-3 Description: Based on a tour of the center the center failed to ensure that an area was available and accessible so that participants have opportunities for supervised outdoor activities and that the area was equipped with appropriate seasonal outdoor furniture.
Evidence: The licensing inspector observed that the center did not have outdoor furniture to allow participants opportunities for supervised outdoor activities. The center director stated that he would purchase seasonal outdoor furniture.Plan of Correction: The Director purchased appropriate outdoor furniture to ensure furniture was in place prior to facility being licensed.
