Comments:

This inspection was conducted by licensing staff using an alternate remote protocol necessary due to a state of emergency health pandemic declared by the Governor of Virginia. A mandated monitoring inspection was initiated on 5/28/2020. The Administrator was contacted by email to initiate the inspection. The facility has voluntarily closed for the short term for the safety and care of the participants and staff. The inspector emailed the Administrator a list of items required to complete the inspection. The inspector reviewed three participant records, and three staff records, local fire and health inspection and other documentation submitted by the facility to ensure documentation was complete. All background checks of staff hired after the most recent inspection were reviewed. The information gathered during the inspection determined no violations with applicable standards or law.
