A monitoring inspection was initiated on 8/16/21 and concluded on 8/19/21. The administrator was contacted by telephone to initiate the inspection. The administrator reported that the current census was 19. The inspector emailed the administrator a list of items required to complete the remote documentation review portion of the inspection. The inspector reviewed two resident records, two staff records, the activities menu, the menu, staff schedules, etc. submitted by the facility to ensure documentation was complete. The inspector conducted the on-site portion of the inspection on 8/19/21. An exit interview was conducted with the Administrator on the date of the inspection, where findings were reviewed and an opportunity was given for questions, as well as for providing any information or documentation which was not available during the inspection.
Information gathered during the inspection determined non-compliances with applicable standards or law, and violations were documented on the violation notice issued to the facility.

Standard #: 22VAC40-73-610-B Description: Based on observations made during the tour of the building, the facility failed to have the menu for the current week dated.
EVIDENCE:
1. The menu posted in the facility had the dates for 2020, not the current year of 2021.Plan of Correction: Menus will be dated to reflect current week. Menus will remain color coded with corresponding calendar posted in dining room with daily menu visible on board. In the future, menus will be dated with the current week visible and remain color coded. Administrator will monitor for compliance. [sic]
Standard #: 22VAC40-73-680-D Description: Based on observations made during the medication cart audit, the facility failed to administer medications consistent with the standards of practice outlined in the current registered medication aide curriculum approved by the Virginia Board of Nursing.
EVIDENCE:
1. Resident #1 is prescribed Levemir, Dorzolamide-Timolol Eye Drops, and Erythromycin 0.5% Eye Ointment among other medications. These three medications were opened and had been used but were not labeled with an open date.
2. The top drawer of the facility medication cart contained more than 20 different medications that were not labeled with an open date such as eye drops, ear drops, nasal sprays.Plan of Correction: Medication Management Manual will be updated to include specific instruction detailing which medication must be labeled with an open date to ensure no out of date medication is distributed to any resident of the facility. Additionally, all Registered Medication Aides will receive in-house training verifying knowledge and demonstrating practice of standard. All medication requiring an open date will be labeled correctly. In the future, Registered Medication Aides will receive routine training to ensure proper practice. Administrator will monitor. [sic]
