Unannounced monitoring inspections were conducted on 8/16/23 and 8/25/23 to follow-up on a facility reported incident. Interviews were conducted, facility documentation observed, one resident record was reviewed, and one staff record was reviewed. Exit meeting held. The evidence gathered during the investigation did not support the self-report of non-compliance with standard(s) or law.
