Comments:
A non-mandated complaint inspection was initiated on 07/12/2021nd conclude ad on 07/16/2021. A complaint was received by the department regarding allegations in the areas of the physical plant. The administrator was contacted by telephone to conduct the investigation. The licensing inspector emailed the administrator a list of documentation required to complete the investigation. The licensing inspector conducted an on-site observation at the facility on 07/12/2021.
The evidence gathered during the investigation supported the allegations of non-compliance with standards or law, and violations were issued. Any violations not related to the complaint but identified during the course of the investigation can be found on the violation notice.

Standard #: 22VAC40-73-450-C
Complaint related: No
Description: Based on reviews of resident records the facility has failed to document needs on the Individualized Service Plan (ISP) that were identified from the Uniform Assessment Instrument (UAI).
EVIDENCE:
1. Resident #6 has a UAI dated 03/04/2021. The ISP is dated 03/04/2021. The UAI identifies meal preparation, housekeeping, phone use, laundry, transportation and shopping as a documented need. The ISP does not address any of these needs.

Plan of Correction: The provider's response for the plan of correction for this violation was not received as of 07/26/2021 and will not appear on this violation notice.

Description: Based on observation and staff interviews, the facility has failed to ensure that personal assistance and care are provided to each resident as necessary so that the needs of the resident are met.
EVIDENCE:
1. Staff #1 and #2 state they have a very difficult time getting Resident #4 to take a shower and it has been as long a month between his showers in the past.
2. Staff #2 stated he had bribed Resident #4 to take a shower on multiple occasions, rewarding the resident with money.

Plan of Correction: Resident was admitted for his behavior. We continue trying by all means (Administrator will monitor). [sic]

Complaint related: Yes
Description: Based on observations made during the tour of the building, the facility failed to ensure that each resident had sufficient bed and bath linens in good repair so that residents always have clean sheets, pillowcases, blankets, bedspreads, and waterproof mattress covers.
EVIDENCE:
1. Resident #1, 3, and 4 had unclean sheets. Resident #3 had dirt and dust particles near the bottom of the sheets on the bed, Staff #1 stated this was because she often slept with her shoes on. Resident #1 had visible red, rust colored ? inch spots near his pillowcase, these appeared to be dried blood and old blood stains. Resident #4 had discolored sheets, Staff #1 states this is because he will not take a shower for a month at a time.

Plan of Correction: The staff is doing laundry once a week and as needed. Administrator will monitor and have resident to sign every week. [sic]

Description: Based on observations made during the tour of the building, the facility failed to ensure that bed and bath linens shall be changed at least every seven days and more often if needed.
EVIDENCE:
1. Resident #1 stated that his bed linens were not changed at least every seven days, he felt it was
closer to a month in between bed linens being changed.

Plan of Correction: Linens are changed every week and as needed. Administrator will get resident sign weekly. [sic]

Complaint related: Yes
Description: Based on observations made during the tour of the building, the facility failed to ensure the building was kept free of infestations of insects and vermin and their breeding places.
EVIDENCE:
1. Residents #1 and 2 had visible bed bugs on their mattress when licensing inspector and Staff #1 pulled back the fitted sheet. The bugs were inactive and did not appear to be alive.
2. Staff #1 stated she treats resident rooms on a daily basis with Ortho Bed Bug Spray. The
manufacture suggest using this product one time every two weeks.

Plan of Correction: There are no live bugs. We will (administrator or staff) will monitor. [sic]
