Comments:
Two licensing inspectors conducted a complaint investigation on 12/07/2021 beginning at 12:20 pm and concluding at 1:00 pm. The allegations being reviewed were resident accommodations. Based on a review of the evidence,, the complaint was determined to be valid. One violation is being cited from this complaint investigation. An exit interview was held with the administrator of the facility on 12/04/2021. The opportunity to locate items that were not available during the inspection was given. Please complete the columns for "description of action to be taken" and "dates to be corrected" for each violation cited on the violation notice, and then return a signed and dated copy to the licensing office within 10 calendar days (12/25/2021) of receipt. Description: Based on observations made and interviews with residents, the facility failed to keep the building free from infestations of insects, and the grounds kept free of their breeding places.
EVIDENCE:
1. The LI observed what appeared to be raised red bites on both of Resident #1's hands and forearms. Resident # 1 stated ?these are bed bug bites. These bugs are in my bed and bite me.? Resident # 1 stated she showed Staff # 1 a few days ago and she has not been to the doctor to address the bites. She stated she was told by Staff # 1 to put lotion on the bites to help with itching. Resident # 1 also stated that Staff #1 had sprayed her bed with something in attempt to kill the bugs.
2. The LI observed raised red bites on Resident #2?s left forearm. Resident # 2 is the roommate of Resident #1. Resident #2 stated ? I have bugs that crawl in my bed and I kill as many as I can so they will not bite me.?
3. The LI observed what appeared to be bed bugs stuck to Resident #2?s sheets as well as a bed bug crawling through Resident # 2 bed sheets.

Plan of Correction: The bugs are in central. There are several new methods used to get rid of bugs including heat treatment. [sic]
