Comments:
A non-mandated complaint inspection was initiated on 08-05-2021 and concluded on 08-23-2021. A complaint was received by the department regarding allegations in the areas of Resident Care and Related Services. The Administrator was contacted by telephone to conduct the investigation. The licensing inspector emailed the Administrator a list of documentation required to complete the investigation. The evidence gathered during the investigation did not support the allegations of non-compliance with standards or law.

Description: Based on record review and interview, the facility failed to report to the regional licensing office within 24 hours any major incident that has negatively affected or that threatens the life, health, safety, or welfare of any resident.
Evidence:
1. ?Progress Notes? documented:
A. On 07-24-2021, resident #2 ?fell to the ground unresponsive? called 911 to report unexpected death.?
B. On 07-28-2021, resident #3 ?was observed sitting on bathroom floor?? The resident was sent to the emergency room and was admitted to the hospital. Resident #3 was admitted to the hospital due to ?slip and fall and kidney stones.?
C. The regional licensing office did not receive incident reports from the facility within 24 hours regarding the aforementioned incidents.
2. Staff #2 acknowledged the aforementioned incidents were not reported to regional licensing office within the required timeframe.

Plan of Correction: Resident Care Director re-in-serviced on AL regulation 40-73-(2)-70-A Incident Reporting. Each facility shall report to the regional licensing office within 24 hours any major incident that has negatively affected or that threatens the life, health, safety, or welfare of any resident.
