Comments:

Licensing Inspector (LI) conducted unannounced complaint investigation on 10/28/2021 regarding infection control. LI reviewed facility policy, signage and training documentation. Staff was observed and interviewed. Spoke with Executive Director and Wellness Director. Complaint regarding Standard 100 is deemed valid as a preponderance of evidence gathered during the investigation supported the allegations. Exit interview conducted on 11/29/2021 and the violation notice regarding the standards deemed valid was provided for correction. Areas of non-compliance are identified on the violation notice. Please complete the "plan of correction" and "date to be corrected" for each violation cited on the violation notice and return to the licensing office within 10 calendar days. Please specify how the deficient practice will be or has been corrected. Just writing the word "corrected" is not acceptable. The plan of correction must contain: 1) steps to correct the non-compliance with the standard(s), 2) measures to prevent the non-compliance from occurring again, 3) person(s) responsible for implementing each step and/or monitoring any preventative measure(s), and 4) date that that plan of correction will be completed. Description:	Based on observation and documentation, facility failed to implement an infection control program addressing the control of disease and infection that is consistent with the federal Centers for Disease Control and Prevention (CDC) guidelines. EVIDENCE: On 10/28/2021, Licensing Inspector (LI) and Staff 1 observed that facility policy, signage and training regarding mask wearing was not implemented when Staff 2 was observed serving lunch to residents with a mask lowered below her nose.
Plan of Correction:	All staff will be retrained on properly wearing masks. Executive Director will walk through the community and randomly check team members for appropriately wearing masks. Masks will be available if anyone is noted to have an improper fitting mask.
