Comments:

A renewal inspection was initiated on 08/11/2020 and concluded on 08/12/2020. The Administrator was contacted by telephone to initiate the inspection. The Administrator reported that the current census was 74. The inspector emailed the Administrator a list of items required to complete the inspection. The inspector reviewed 74 resident records, 74 staff records, and additional documents submitted by the facility to ensure documentation was complete. Information gathered during the inspection determined non-compliance(s) with applicable standards or law, and violations were documented on the violation notice issued to the facility. Description:	Based on resident record review the facility failed to ensure the comprehensive individualized care plan included the expected outcome, time frame for expected outcome and date outcome achieved. Evidence: On 08/11/2020 while reviewing Individualized Service Plan's for 4 out of 4 resident's, it was observed that the Individualised Service Plan's provided to the Licensing Inspector during the inspection, did not include the expected outcome and time frame nor date outcome achieved.
