This inspection was conducted by licensing staff using an alternate remote protocol necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A renewal inspection was initiated on 8/5/2020 and concluded on 8/12/2020. The Administrator was contacted by telephone to initiate the inspection. The Administrator reported that the current census was 61. The inspector emailed the Administrator a list of items required to complete the inspection. The inspector reviewed 4 resident records, 4 staff records, staff schedules, Health Care Oversight, Fire Inspection Record, Health Department Inspection Record, Fire and Emergency drills, Oversight by dietitian or nutritionist, and Staff Sworn Disclosure Statements and Criminal Record Checks for new hires since last inspection.
Information gathered during the inspection determined non-compliance(s) with applicable standards or law, and violations were documented on the violation notice issued to the facility.

Standard #: 22VAC40-73-250-D Description: Based on record review and discussion, the facility failed to receive an initial tuberculosis examination and report on or within seven days prior to the first day of work documenting the absence of tuberculosis in a communicable form as evidenced by the completion of the current screening form published by the Virginia Department of Health or a form consistent with it.
Evidence: 1.Staff#1?s first day of work was dated 4/6/20 and the Tuberculosis (TB) screening was completed on 1/19/2020. 2. Staff #2 acknowledged that the facility did not have a TB screening for Staff #1 on or within seven days prior to the first day of work.3. Staff #3?s first day of work was dated 6/3/19 and the Tuberculosis (TB) screening was completed on 3/9/2016. 4. Staff #2 acknowledged that the facility did not have a TB screening for Staff #3 on or within seven days prior to the first day of work.Plan of Correction: 1. Staff #1 obtained a new TB screening on 8/27/20. Staff#3 obtained a TB screening /Chest X-Ray on 8/25/20.
2. Administrator educated Human Resource staff on DSS regulations pertaining to initial Tuberculosis examination/screening and that it must be completed on or within 7 days prior to the first day of work.
3. The Human Resource department shall conduct an audit of all employees that have been employed within the last 12 months to ensure they received their initial tuberculosis examination/screening.
4. A new tuberculosis examination shall be obtained on or within 7 days prior to the first day of work for any new employee hired.
