Comments:

A renewal inspection was completed on 12/15/2021. The administrator was on site during the inspection and reported a current census of 17 residents. The inspector reviewed two (2) resident records, two (2) staff records, required postings, the emergency food supply, medication administration and inspected the building for compliance. An exit interview was conducted with the Administrator where findings were reviewed and an opportunity was given for questions, as well as for providing any information or documentation which was not available during the inspection. Information gathered during the inspection determined non-compliance(s) with applicable standards or law, and violations were documented on the violation notice issued to the facility.

Description:	Based on an inspection at the facility on 12/15/21, the facility failed to obtain at least an annual health inspection report by the appropriate health official Evidence: The most current health inspection report is dated 3/7/2019.

Standard #:	22VAC40-73-660-A-2
Description:	Based on an inspection of medication administration on 12/15/2021, schedule II drugs that are subject to abuse are not kept in a separate locked storage compartment (e.g., a locked cabinet within a locked storage area or a locked container within a locked cabinet or cart). Evidence: An inspection of the medication storage area on 12/15/2021 found schedule 11 drugs in the storage area with other medications, not in a separate locked container.

Standard #:	22VAC40-73-940-A
Description:	Based on an inspection at the facility on 12/15/21, the facility failed to obtain at least an annual fire inspection report by the appropriate fire official. Evidence: The facility has not had an annual fire inspection. The most current inspection was dated 2019.
Plan of Correction:	They did follow up at the other facilities but has not made it to Harris Manor.. We are on the list to be inspected.
