A non-mandated complaint inspection was initiated on 2/19/21 and concluded on 7/15/21. A complaint was received by the department regarding an allegation in the area of: Resident Care and Related Services. The administrator was contacted by telephone to conduct the investigation. The licensing inspector emailed the administrator a list of documentation required to complete the investigation. The licensing inspector conducted on-site observations at the facility on 6/28/21 and 7/15/21.
The evidence gathered during the investigation supported the allegation of non-compliance with standards or law, and a violation was issued. Any violations not related to the complaint but identified during the course of the investigation can be found on the violation notice.

Standard #: 22VAC40-73-470-C Complaint related: Yes Description: Based on record review, the facility failed to ensure that services are provided to prevent clinically avoidable complications, including dehydration.
Evidence: Resident progress notes state that Resident #1 was sent to the hospital on 1/4/21 to rule out GI blockage, as the resident had not been eating/drinking for 24 hours. The progress notes indicate that Resident #1 began refusing food/water on 1/2/21 at dinner, as she spit out food and verbalized that she didn't want to eat. On 1/3/21, Resident #1's progress notes stated that the resident refused breakfast and lunch, spit out food, and she was not swallowing. The 1/3/21 note requests staff to follow-up at dinner, and to contact the physician if the resident is continuing to refuse food/water. The facility's 24 hour log documented that Resident #1 continued to refuse to eat dinner, but the physician was not contacted until 1/4/21, as the resident continued to be unable to swallow food/drink.
Hospital documentation states that Resident #1 was unresponsive and non-verbal in the ER. She was admitted to the hospital and her diagnoses included Sepsis due to E. Coli UTI, Dehydration, and Pneumonia due to COVID-19.Plan of Correction: Effective 8/1/21, the oncoming RSS will review the 24 hour report and note any follow-up that is required. The oncoming RSS will initial in red and mark as either completed or other action that was taken.
RSC will review the 24 hour report during stand up to ensure follow-up was completed or action taken. The RSC will initial the 24 hour report as it was reviewed.
